A Letter of Consent and Power of Attorney for a Minor Child (under 20 years old) to Obtain a Passport

For requests to issue a power of attorney, consent form, or signature authentication, you must appear in person to sign in front of the officer. Documents cannot be submitted by mail for legalization. These services are available only on Tuesdays and Wednesdays, between 10:00 a.m. and 2:00 p.m. An appointment is required. There is no fee for issuing the Letter of Consent and Power of Attorney for a minor child to apply for a passport.

1. A Letter of Consent for a Minor Child to Obtain a Passport

A Letter of Consent is a document in which a father or mother gives their consent by signing, allowing their minor child (under 20 years old) to obtain a passport for international travel. This is used in cases where the father or mother cannot personally travel to sign the passport application in front of officials at the Ministry of Foreign Affairs.

Steps to Complete the Process:

  3. Supporting Documents of parents and child: *Prepare 2 copies of each document. Documents must be prepared for both the father’s and mother’s sides.

For Parents with Thai Nationality:

    • Original Thai national ID card(s) of the father and/or mother.
    • Copy of the father and/or mother’s valid Thai national ID card (2 copies).
    • Copy of the Thai household registration (Tabien Baan) for the father and/or mother, plus 2 copies.
    • Copies of any relevant documents (2 copies each), such as:
      • Name change certificates.
      • Surname change certificates.
      • Marriage certificates.
      • Divorce certificates and divorce records.
      • Sole custody documents (Por.Kor.14).
      • Death certificates.
      • Adoption certificates.

For Parents without Thai Nationality (Swedish or Latvian):

    • Original valid passports of the father and/or mother.
    • Copy of the father and/or mother’s valid Thai national ID card (2 copies).
    • Copies of any relevant documents (2 copies each), such as:
      • Name change certificates.
      • Surname change certificates.
      • Marriage certificates.
      • Divorce certificates and divorce records.
      • Sole custody documents.
      • Death certificates.
      • Adoption certificates.

Child's Documents:

    • Copy of the child’s Thai birth certificate (2 copies).
    • Copy of the child’s Thai household registration (Tabien Baan) (2 copies).
    • Copy of the child’s valid Thai national ID card, if available (2 copies).
    • Copy of the child’s current or expired passport, if available (2 copies).
    • Copies of other relevant documents (2 copies each), such as name change or surname change certificates.

 

2. Power of Attorney for a Minor Child to Obtain a Passport

A power of attorney is required in addition to the consent form when the minor (under 15 years old) must be accompanied by another person (age 20 or older) to apply for a passport. This power of attorney is necessary when neither parent can accompany the child to submit the application in person.

  3. Supporting Documents of Parents, Child, and Representative: *Prepare 2 copies of each document. Documents must be prepared for both the father’s and mother’s sides.

For Parents with Thai Nationality:

    • Original Thai national ID card(s) of the father and/or mother.
    • Copy of the father and/or mother’s valid Thai national ID card (2 copies).
    • Copy of the Thai household registration (Tabien Baan) for the father and/or mother (2 copies).
    • Copies of any relevant documents (2 copies each), such as:
      • Name change certificates.
      • Surname change certificates.
      • Marriage certificates.
      • Divorce certificates and divorce records.
      • Sole custody documents (Por.Kor.14).

    • Copy of the father or mother’s valid Swedish or Latvian passports (2 copies).

      • Divorce certificates and divorce records.
      • Sole custody documents.
      • Death certificates.
      • Adoption certificates.

Child's Documents:

    • Copy of the child’s Thai birth certificate (2 copies).
    • Copy of the child’s Thai household registration (Tabien Baan) (2 copies).
    • Copy of the child’s valid Thai national ID card, if available (2 copies).
    • Copy of the child’s current or expired Thai passport, if available (2 copies).
    • Copies of other relevant documents (2 copies each), such as name change or surname change certificates.

For the Appointed Representative:

    • Copy of the valid Thai national ID card of the appointed person (who must be over 20 years old) (2 copies).

 

*After preparing all the required documents, please schedule an appointment in advance (CLICK HERE to make appointment) and bring the documents to the consular officer at the Embassy to issue the Letter of Consent and Power of Attorney. Once you have obtained the Letter of Consent and Power of Attorney, you may submit them to the officials at the Ministry of Foreign Affairs to proceed with the application for your child’s Thai passport.
